What You'll Do:
  • Support youth and families throughout the permanency process
  • Serve as a key liaison between youth, caregivers, state agencies, and community partners
  • Utilize trauma-informed, strength-based, and youth-guided practices
  • Facilitate family engagement and community connections
  • Participate in treatment planning, court hearings, reviews, and case management
  • Support youth with life skills, goal setting, and successful transitions
  • Coordinate visits, appointments, meetings, and permanency-related activities
  • Collaborate closely with clinical and permanency team members

Qualifications
  • Bachelor's degree in Social Work, Psychology, Education, or a related field
  • Valid driver's license
  • Must be 21 years of age or older
  • Ability to successfully complete background and pre-employment screenings
