What You'll Do:- Support youth and families throughout the permanency process
- Serve as a key liaison between youth, caregivers, state agencies, and community partners
- Utilize trauma-informed, strength-based, and youth-guided practices
- Facilitate family engagement and community connections
- Participate in treatment planning, court hearings, reviews, and case management
- Support youth with life skills, goal setting, and successful transitions
- Coordinate visits, appointments, meetings, and permanency-related activities
- Collaborate closely with clinical and permanency team members

- Bachelor's degree in Social Work, Psychology, Education, or a related field
- Valid driver's license
- Must be 21 years of age or older
- Ability to successfully complete background and pre-employment screenings
