OverviewCorewell Health South Homecare provides high‑quality in‑home care to patients requiring short‑term assistance with daily life activities and specialized treatment modalities. Nurses are assigned to specific territories to reduce driving time and enhance meaningful patient interactions.
Scope of WorkThe LPN, within the ANA scope of practice, employs the nursing process to deliver professional care. The LPN follows patient‑family centered care principles to assess, plan, implement, evaluate, and discharge patients. They work under the direction of the Nurse Leader, following Corewell Health policies and procedures, and recognize the impact of patient/family partnerships on outcomes. The LPN collaborates with patient and family to develop goals, document teaching strategies, and record education in the health record.
Responsibilities- Provide quality patient care in home health settings.
- Assess and plan individualized care with patient/family input.
- Implement and evaluate care plans.
- Document education and learning appropriately.
- Follow Corewell Health policies and procedures.
- Collaborate with the Nurse Leader and care team.
- High School Diploma or equivalent.
- Completion of an approved LPN nursing program.
- Licensed LPN in the State of Michigan.
- Driver's license in the State of Michigan.
- BLS certification within 90 days of hire.
- Graduates from a non‑accredited nursing school must have at least one year of acute care experience or at least two years of experience in a licensed health‑care role and must have passed the NCLEX exam.
- One year of relevant clinical experience, recent clinical experience in home health and/or other clinical settings.
- Associate's Degree or equivalent.
- Graduate of an approved nursing program.

Primary location:
Professional Building Meadowbrook, 2550 Meadowbrook Rd, Benton Harbor. Full‑time, variable shift. Weekly scheduled hours: 40. Work time: 8 a.m. to 4:30 p.m., Monday to Friday; variable weekends.
