New Horizon Counseling Center is seeking a Health Home Care Manager to join our team and help individuals with medical, behavioral health, and social needs live successfully in the community. This is a hands-on, community-based role that emphasizes outreach, advocacy, and care coordination.
You will assess client needs, develop care plans, coordinate services with primary care, behavioral health, and housing resources, and help clients access benefits such as Medicaid, SSI/SSD, and SNAP.
