Licensed Practical Nurse (LPN) – Wound Care

Schedule:

8:00 AM – 4:30 PM

• Contribute to patient assessment by collecting data for RN analysis.
• Perform plan of care interventions, including specimen collection and wound care; may administer medication.
• Document patient care and assist RN in development of the plan of care.
• Assist patients with daily living activities: personal hygiene, elimination, nutrition, and ambulation.
• Monitor, document, and communicate patient status and responses to interventions with RN, physician, patient, and family.
• Advocate for the physical and emotional well‑being of the patient.
• Participate in performance improvement and continuing education.
• Provide quality patient care and work flexible shifts/units as needed.
• Maintain regular and reliable attendance.
• Perform other duties as assigned.

• Associate’s Degree – Required.
• Current LPN license in the state.
• At least 1 year of wound care experience.
• Critical‑thinking skills, decisive judgment, ability to work with minimal supervision.
• Ability to work in a stressful environment and take appropriate action.
• Certification: BCLS or receive within one week of hire/transfer.
• Collaborate with RNs and providers, input into development and maintenance of nursing care plan, make and document nursing observations.
