Family Practice Associates is seeking a dependable, compassionate, and organized Licensed Practical Nurse (LPN) Care Coordinator to join our busy family practice team.

The Care Coordinator plays an important role in helping patients transition safely between healthcare settings, supporting providers with ongoing patient care, and improving health outcomes through quality initiatives. This position will work closely with providers, clinical staff, patients, and outside healthcare organizations to ensure appropriate follow-up and continuity of care.

The ideal candidate is a self-motivated team player who is comfortable working independently, managing multiple priorities, communicating effectively with patients, and following through on tasks in a fast-paced medical practice.

• Contact patients following hospital admissions, inpatient stays, emergency room visits, and other significant healthcare encounters to facilitate timely follow-up.
• Perform and document medication reconciliation
  , identifying discrepancies and communicating concerns to the provider as appropriate.
• Contact patients to assess and encourage medication adherence and assist with identifying barriers to treatment.
• Coordinate and schedule appropriate follow-up appointments, diagnostic testing, referrals, and other recommended care

• Participate as an integral member of the practice's Quality Team
  .
• Identify and assist in closing patient care gaps and preventive health needs.
• Monitor patient follow-up and assist in ensuring recommended care is completed.
• Make patient outreach calls and document all appropriate communication and interventions in the electronic medical record.
• Assist with population health and quality-improvement initiatives, including tracking and following up on patients who may benefit from additional services.
• Communicate effectively with patients, caregivers, providers, pharmacies, specialists, hospitals, and other healthcare organizations as appropriate.
• Maintain patient confidentiality and comply with HIPAA and all applicable laws, regulations, and practice policies

• Provide compassionate, professional, and respectful patient interactions.
• Perform other duties within the scope of the LPN license and as assigned by practice leadership.

Current, active, and unrestricted Licensed Practical Nurse (LPN) license in the State of Arkansas.
