Clinic Care Coordinator LPN​/RN; FM

Care Team Coordinator

Work with the current staff to complete the care team in hopes to better improve patient care and the patient's overall experience. This is a part-time position that will report in clinic 2 to 3 times a week reporting to Dr. Mary Massary.

• Promotes patient self-management and empowers patient/ families to achieve maximum levels of wellness and independence
• Responsible for working with patient and patient's care team to coordinate change readiness, needs assessment and develop and individualized treatment care plan
• Collaborates with the patients, physician, and other care teams' members in assessing the patient's progress toward individual health care goals
• Assesses barriers when patient has not met treatment/ goals, is not following treatment plan of care, or has not kept important appointments
• Perform other clinical nurse duties as needed

• Graduation certificate from accredited LPN / RN School of Nursing
• 2 years related experience and/or training
• LPN or RN active license
• Prior Family Medicine experience preferred, but not required
