Clinic Care Coordinator LPN/RN (FM)

Work with the current staff to complete the care team in hopes to better improve patient care and the patient’s overall experience. This is a part-time position that will report in clinic 2 to 3 times a week reporting to Dr. Mary Massary.

• Promotes patient self-management and empowers patient/families to achieve maximum levels of wellness and independence.
• Responsible for working with patient and patient’s care team to coordinate change readiness, needs assessment, and develop an individualized treatment care plan.
• Collaborates with patients, physicians, and other care teams’ members in assessing the patient’s progress toward individual health care goals.
• Assesses barriers when patient has not met treatment goals, is not following treatment plan of care, or has not kept important appointments.
• Perform other clinical nurse duties as needed.

• Graduation certificate from an accredited LPN/RN School of Nursing.
• 2 years related experience and/or training.
• Active LPN or RN license.
• Prior Family Medicine experience preferred, but not required.
