Role
The Primary Care Patient Care LPN works under the primary care operations team to deliver safe, effective, and patient-centered care in collaboration with providers and the clinical team.
Key ResponsibilitiesPartner with patients and staff to drive quality outcomes, perform duties consistent with organizational standards, and support systematic care delivery.
QualificationsGraduate of an accredited nursing program; RN or LPN license in the relevant state; preferred 6 months clinical experience, ideally in primary care.
Physical & Certification RequirementsAbility to lift, push, and pull 50 pounds; prolonged standing and walking; certifications include BLS, with additional certifications based on unit needs, such as ACLS or PALS.
Skills & AttributesStrong clinical skills, ability to work effectively in a team, physical stamina, and adherence to safety standards.
