Our multi-disciplinary care teams incorporate the expertise of medical, dental, behavioral health, pharmacy, and care navigation professionals to provide patients with comprehensive care designed to help them achieve their health goals. While our work is fast‑paced, our teams enjoy being able to work collaboratively to support patients.
ScheduleTwo‑week rotating schedule consisting of 10‑hour shifts, with every other weekend off.
- Week 1 – Sunday 10 a.m. – 8 p.m., Monday 10 a.m. – 8 p.m., Tuesday OFF, Wednesday 10 a.m. – 8 p.m., Thursday 10 a.m. – 8 p.m., Friday OFF, Saturday OFF
- Week 2 – Sunday OFF, Monday OFF, Tuesday 10 a.m. – 8 p.m., Wednesday 10 a.m. – 8 p.m., Thursday OFF, Friday 10 a.m. – 8 p.m., Saturday 10 a.m. – 8 p.m.

Provide primary care services, including assessing, diagnosing, treatment planning and coordination, and managing patients in the clinic, following established standards and practices.
Principal Duties- Complete patient exams/procedures and record findings from complete medical history and physical exams. Order appropriate laboratory and diagnostic procedures. Complete in‑scope procedures as appropriate. Interpret and integrate data to determine preliminary diagnosis and therapeutic plan.
- Conduct one‑on‑one patient teaching and education.
- Participate in Quality Assurance activities and chart reviews.
- Discuss case with Physician and other care team professionals, when/as needed to prepare comprehensive patient care plan.
- Prescribe or recommend drugs or other forms of treatment such as physical therapy, inhalation therapy, or related therapeutic procedures. Administer medications and injections in compliance with state law and clinic guidelines.
- Refer patients to Physician for consultation or to specialized health resources for treatment.
- Demonstrate appropriate work habits which include:
- Arriving on time for clinical sessions and/or meetings.
- Communicating appropriately with patients, other providers, and care team members, and returning patient/provider phone calls as promptly as possible.
- Utilizing appropriate infection control measures.
- Collaborate with Medical Director and other provider staff members in development and periodic updating of standards of care, principles of practice, standing orders and/or clinical protocols (including those governing mid‑level providers).
- As requested, participate with professional and allied health training programs in student and resident rotations.
- Assist in recruitment and orientation of new staff.
- As assigned, attend meetings or make presentations to community groups, associations, or other health organizations as a representative of the organization.
- Performs other duties as assigned.
- Minimum Education
:
Successful completion of an approved nurse practitioner program. BSN, MSN preferred. - Minimum Experience
:
Minimum of 3 years of professional nursing experience in a clinic setting plus at least 1 year of nurse practitioner experience. - Required License
:
Active Nurse Practitioner licensed in the state of Kentucky. Certification by national certifying organization.
