Position: Family Nurse Practitioner- (Evening / Weekend Clinic)

Our Evening & Weekend access schedule creates an alternative work schedule which allows for 4-day weekends every other week
, while extending access to care for health center patients. This Primary Care APRN position cares for patients in the Berea Primary Care Clinic located inside CHI - St. Joseph - Berea.

BASIC FUNCTION

Nurse Practitioner provides primary care services, including assessing and managing patients in the clinic, following established standards and practices.

PRINCIPLE DUTIES
  • Perform a complete physical exam, obtain complete medical history and record findings. Order appropriate laboratory and diagnostic procedures. Interpret and integrate data to determine preliminary diagnosis and therapeutic plan.
  • Conduct one on one patient teaching and education. Educate patients regarding health and illness prevention.
  • Order, interpret, and evaluate diagnostic tests to identify and assess patient’s clinical problems and health care needs.
  • Participate in Quality Assurance activities and chart reviews.
  • Record physical findings, and formulate plan and prognoses, based on patient’s condition. Keep current and accurate records in accordance with clinic medical protocol and provide adequately detailed information to allow accurate and truthful coding of diagnosis and services provided.
  • Discuss case with Physician and other health professionals, when/as needed to prepare comprehensive patient care plan.
  • Prescribe or recommend drugs or other forms of treatment such as physical therapy, inhalation therapy, or related therapeutic procedures.
  • Refer patients to Physician(s) for consultation or to specialized health resources for treatment.
  • Administer medications and injections in compliance with state law and clinic guidelines. Suture minor lacerations.
  • Instruct patient/family regarding medications and treatments.
  • Recommend community resources to meet patient and family needs.
  • Collaborate with physicians in managing acute and long‑term medical needs of patients. Provide monitoring and continuity of care between physician visits. Maintain and review patient records, charts and other pertinent information. Post tests and examination results. Triage patient telephone calls and provide consultation.
  • Maintain patient confidentiality.
  • Perform other duties as assigned.
  • ACCOUNTABILITY

    Reports to the Chief Medical Officer.

    SUPERVISORY RESPONSIBILITIES

    Nurse Practitioner may supervise as preceptor to a nurse practitioner student per organizational guidelines. Employee carries out supervisory responsibilities in accordance with the organization’s policies and applicable laws. Responsibilities include instructing students; planning, assigning, and directing work; appraising performance; addressing complaints and resolving problems.
