Reporting to the Care Manager, the Care Management Support Associate will serve as a member of the Care Management team within Counseling Services, providing non-clinical support, resource navigation, and referral assistance to Berea College students. The position will assist students in identifying and addressing basic needs and other barriers that may affect their academic success, sense of belonging, and progress toward graduation.

The Associate will receive comprehensive training, shadowing, and progressively independent, supervised experience in Care Management practices. Working within established Care Management processes and under the guidance of the Care Manager, the Associate will meet with students to identify needs, connect students with campus and community resources, facilitate referrals, and provide follow-up support.

The Associate will work collaboratively with the Care Manager and other campus partners to support students and will refer students to the Care Manager or other appropriate professional resources when needs fall outside the scope of non-clinical Care Management. The position will also assist with Care Management program development, student outreach, and the work of the Health Education Advocacy Resource Team (HEART).

Through structured training, supervision, and experiential learning, the position is designed to provide a recent Berea College graduate with opportunities to develop professional skills in student support, resource navigation, advocacy, program development, and higher education.

Direct Student Support – 65% (26 hrs./week)
  • Participate in student intake appointments and conduct basic needs assessments using established Care Management processes, tools, and protocols under the guidance and supervision of the Care Manager.
  • Facilitate collaborative appointments with students to identify needs, goals, and barriers to success, and assist students in developing individualized service plans.
  • Connect students to appropriate campus and community resources to address identified needs, including housing, food access, social benefits, financial resources, clothing or durable goods, and other basic needs.
  • Provide resource navigation, referral coordination, follow-up communication, and other non-clinical support to students within the scope of Care Management services.
  • Serve as an initial point of contact for students seeking support and promptly connect students with the Care Manager or other appropriate professional resources when the concerns exceed the scope of non-clinical Care Management.
  • Advocate for students and assist them with navigating institutional and external service systems within the scope of the position.
  • Maintain regular communication with assigned students and provide follow-up support and monitor progress toward identified goals and needs.
  • Develop increasing independence in managing assigned clients through structured training, supervision, consultation, and ongoing professional development.
Health Education Advocacy Resource Team – 25% (10 hrs./week)
  • Assist with programmatic development and day-to-day operations in collaboration with the Health Education Advocacy Resource Team (HEART) Labor Students and Supervisor.
  • Coordinate completion of the 2024 Work College Consortium grant awarded to HEART, with guidance from the HEART Supervisor and Care Manager, including preparation for the final public presentation.
Administrative – 10% (4 hrs./week)
  • Maintain detailed and accurate case files, including but not limited to, tracking client contacts, referrals, and care management progress in accordance with FERPA, HIPAA, and departmental standards.
