Essential Functions:
  • Demonstrate the values and guiding principles of Ohio Guidestone every day, performing with the highest standards of excellence, integrity and ethics.
  • Review electronic health records to identify clients requiring timely follow-up in accordance with agency protocols, quality standards, and value-based care measures.
  • Identify care gaps and notify clinicians and supervisors of client follow-up needs to promote continuity of care.
  • Monitor completion of required outreach and follow-up activities, communicating with staff as needed until care gaps are resolved.
  • Facilitate timely communication between providers and clients by ensuring appropriate outreach activities are completed.
  • Track quality measures, care gap closure, follow-up activities, and performance metrics using Microsoft Excel and other reporting tools.
  • Maintain accurate tracking spreadsheets, databases, and reports, and prepare routine quality performance reports for leadership.
  • Support adherence to agency quality standards, clinical protocols, payer requirements, and value-based care initiatives across mental health, substance use disorder (SUD), psychiatry, and medical services.
  • Communicate professionally and effectively with clinical staff and supervisors through email and other communication methods to promote timely follow-up.
  • Maintain strict confidentiality of protected health information (PHI) in accordance with HIPAA and agency policies.
  • Assist with quality improvement initiatives, departmental projects, and other administrative duties as assigned.
  • Exhibit positivity, flexibility and a willingness to take on new responsibilities as requested or required.
  • Demonstrate positive leadership, promote a team-based work environment and a culture of belonging.
  • Present the Agency in the most positive light with all internal and external contacts.
Education & Experience:
  • High school diploma or equivalent.
  • Minimum of one (1) year of experience in healthcare, behavioral health, medical office administration, quality support, or a related administrative position.
  • Proficiency in Microsoft Excel, Outlook, and Microsoft Office applications.
  • Associate or bachelor’s degree in healthcare administration, psychology, social work, business administration, or a related field.
  • Experience working with electronic health records (EHRs).
  • Experience in behavioral health, healthcare quality, population health, or value-based care.
Qualifications:

  • Exceptional attention to detail and organizational skills.
  • Ability to perform structured record review with a high degree of accuracy and consistency.
  • Intermediate proficiency with Microsoft Excel, including maintaining tracking spreadsheets, sorting and filtering data, and preparing basic reports.
  • Strong written communication skills, particularly professional email communication.
  • Persistent follow-through and the ability to monitor tasks through completion.
  • Ability to prioritize multiple responsibilities while meeting deadlines.
  • Possess a valid Ohio Driver’s License with a safe driving record and valid insurance.
  • Successfully complete pre-employment screenings, including drug tests, background checks, and any other required assessments, such as a physical examination and TB test.
  • Commitment to maintaining strict confidentiality and safeguarding sensitive client information.
