Nursing Home Social Worker
Runnells Center for Rehabilitation and Healthcare is seeking a full-time Nursing Home Social Worker to work for our Skilled Nursing Facility located in Berkeley Heigths, NJ
Bachelor's in Social Work (BSW) or a Master's in Social Work (MSW) from a CSWE-accredited program required!
Duties Include:
* Maintaining and/or improving the residents' ability to control everyday physical needs and mental / psychosocial needs
* Identifying individual social and emotional needs
* Developing and maintaining individualized care plans
* Participating in the planning of the resident's admission, community re-entry or transfer
* Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident
Requirements:
* Bachelor's degree required. Master's degree strongly preferred.
* LSW or CSW Licensure Required
* EXPERIENCE WITH MDS 3.0 / PHQ-9 / BIMs
* Long-term care / sub-acute experience preferred
