Job Summary

The Department of Pharmaceutical Services provides a full range of services for patients cared for by the Medical Center at UCSF and its providers and payors. These services include, but are not limited to: drug procurement, storage, and security; safe, accurate and timely preparation and distribution of drugs; clinical pharmacy services for patients; development of drug use policies within the Medical Center;

drug information services for clinicians and patients; and participation in technology assessment and quality assurance programs. The Department of Pharmaceutical Services is affiliated with the UCSF Department of Clinical Pharmacy, an academic department within the School of Pharmacy, which has primary responsibility for teaching professional courses in the Pharm.

Position:
Oncology and Infusion Staff II Pharmacist

Responsibilities

Under the Department of Pharmaceutical Services, the Oncology and Infusion Staff II Pharmacist provides pharmaceutical care to patients in all areas where pharmaceutical services for Infusion/Adult Hematology/Adult Oncology are provided within the UCSF Health System. They may also be asked to participate in pharmaceutical care in other sectors of the Department of Pharmaceutical Services as needed. The Oncology and Infusion Staff II Pharmacist performs a wide range of pharmaceutical care functions to assure that drugs prescribed for and used by patients are safe and effective.

These include, for example, obtaining medication histories, reviewing and assessing medication profiles, providing drug information to patients and health care providers, collaborating with clinicians on drugs of choice and therapeutic plans, ensuring access to medications, educating patients on the proper use of medications, and ensuring that patients understand cautions, adverse reactions, and interactions related to their therapy. The Oncology and Infusion Staff II Pharmacist will be part of an interprofessional team consisting of physicians, advanced practice providers, nurses, and other professionals.

The Oncology and Infusion Staff II Pharmacist will be based in the ambulatory or inpatient setting managing injectable, oral, and/or infusion medications for patients with hematologic and oncologic malignancies. Depending on the assigned shift and site, the Oncology and Infusion Staff II Pharmacist may also oversee pharmacy technicians for sterile compounding or facilitate insurance authorizations for medications. The Oncology and Infusion Staff II Pharmacist may also be responsible for the oversight of clinical activities, delivery of services, and the Patient Management Program.

Oversight of insurance authorization and Patient Management Program may be part of pharmacist responsibilities as applicable. Staff are expected to participate in department initiatives to improve quality and safety.

Required Qualifications
  • Doctor of Pharmacy degree (Pharm

    D) or equivalent degree as determined by the National Board of Pharmacy
  • Experience in a hospital setting; may include one year residency in hospital or clinical pharmacy practice and/or one year supervisory; or an equivalent combination of education and experience
  • The flexibility to orient and work at all UCSF Health locations
Preferred Qualifications
  • Completion of an ASHP-accredited PGY1 Pharmacy Residency and/or Fellowship, or at least 3 years of Oncology/Infusion experience.
  • Completion of an ASHP-accredited PGY2 Oncology Pharmacy Residency and/or Fellowship, or at least 3 years of Oncology/Infusion experience.
  • Advanced board certification (e.g., BCOP, BCPPS, or BCPS).
  • Operational experience in Oncology and/or Infusion Pharmacy.
  • Experience in sterile compounding of hazardous and non-hazardous drugs.
License/Certification

Active license to practice Pharmacy in the State of California; must be in good standing with the California State Board of Pharmacy. Active American Heart Association CPR Certification.
