Berkeley Area
In partnership with other clinicians on the case, you will provide skilled services to evaluate and address the needs of individuals in their homes, enabling them to live safely and productively in their home environment. Your goals as a Physical Therapist Case Manager, Level 2 are to restore function to prevent or minimize disability following disease or injury and to help the client reach their maximum performance/function within the limits of their capabilities.
You will evaluate and treat patients using medically prescribed physical therapy programs.
You will also supervise 2 Physical Therapy Assistants (PTAs) to ensure they meet performance standards.
What you’ll do as a Home Health Physical Therapist Case Manager, Level 2- Perform an evaluation in the home and develop a care plan while working with PTAs to follow the plan.
- Help patients with mobility, movement, and regaining independence.
- Create detailed treatment and care plans for each patient.
- Communicate and coordinate with patients’ physicians to ensure quality care.
- Evaluate and report patients’ progress to MD.

- Competitive compensation with achievable incentive bonus program.
- Current unrestricted CA Physical Therapist License.
- 1+ year experience supervising PTAs in a home health environment is a plus.
- Current BLS/CPR Certification.
- Valid CA Driver’s License.
- Knowledge of local community resources is required.
- Knowledge of individual development and human behavior as it relates to illness/injury, and of the influence of culture on health care.
- Knowledge of state and federal home‑care regulatory guidelines preferred.
- Experience in home health care highly preferred.
- Experience with high acuity patients is a plus.
- Experience working with an interdisciplinary team highly desired.

Salary range: $125,000 – $160,000 per year. Plus a $20,000 annual bonus paid quarterly. Starting pay is commensurate with relevant experience above the minimum requirements. 21st Century offers generous bonus incentive plans, comprehensive health benefits and a 401(k) plan with up to 4% match.
