Medical Records Clerk
Location: Administrative Offices Sixth Street - Berkeley, CA 94710
Position Type: Full Time
Salary: $22.00 - $22.00 Hourly
Full time, 40 hours/week, benefit eligible position.
Represented by SEIU-UHW. Salaries and benefits are set by a collective bargaining agreement (CBA). Employees in this position must remain members in good standing of SEIU-UHW, as defined in the CBA.
Life Long Medical Care is a multi‑site Federally Qualified Health Center (FQHC) that provides innovative healthcare and social services to a diverse patient community. The organization has over 15 primary care health centers and delivers integrated services including psychosocial support, referrals, chronic disease management, dental care, health education, and home visits.

- Maintain the medical records system: pull charts for patient appointments, re‑file charts, assemble new charts, file lab reports, repair charts, and locate charts for medical providers and other staff members.
- Assist triage nursing team by pulling charts for triage calls.
- Duplicate immunization records when requested by patients.
- Respond to written requests for patient information and calls from other facilities by pulling charts and forwarding to the appropriate provider in a timely fashion.
- Assist chart preparation personnel by locating results when requested, the day prior to the patient’s appointment.
- Receive daily incoming mail, distribute with charts as needed to appropriate recipients.
- Manage retrieval of charts from storage, purge charts, and manage storage of purged charts.
- With instruction from providers, arrange for copying patient records requests and/or complete records requests from outside sources, adhering to timelines for completion.
- Perform other duties as assigned by the Medical Records Supervisor.
- Ability to prioritize work and multitask.
- Ability to read and comprehend instructions, procedures, and emails.
- Strong clerical and computer skills, with experience in practice management systems.
- Excellent internal and external customer service skills and the ability to maintain a positive attitude under pressure.
- Strong organizational, administrative, and problem‑solving skills; flexible and adaptive to change.
- Ability to seek direction/approval for essential matters yet work independently with little onsite supervision, using professional judgment and diplomacy.

- High school diploma or GED.
- Two years of experience in medical records.
- One year of experience using an electronic health records system.
- Knowledgeable in basic medical terminology.
- Proficient in Microsoft Office Suite.
- Bilingual in English and Spanish.
- Experience in a community health care setting.
- Experience with Epic Systems EHR.
