OverviewLife Long Medical Care has an exciting opportunity for a Registered Nurse II - Triage to join our multidisciplinary team to provide patient care via telephone triage and our electronic health record. This is a Hybrid/Remote role. Candidate must live in California and have a CA RN License. The ideal candidate must have a commitment to providing high quality primary care for complex patients, dedicated to building a collaborative team model of care, ideally experience working with Covid-19 related care and an interest in serving a diverse patient population.
Communication is a key component of a triage nurse's job. In addition to talking with patients who may be experiencing a medical emergency, triage nurses relay patients' conditions to other medical personnel, including doctors and nurses, and provide information to patients.
This position is represented by SEIU-UHW. Salaries and benefits are set by a collective bargaining agreement (CBA), and an employee in this position must remain a member in good standing of SEIU-UHW, as defined in the CBA.

- Participate in the coordinated care for patients seeking care via telephone and MyChart messages.
- Follow up with patients and ensure communication with the primary care team.
- Coordinate with Care Transitions team for all hospitalized patients to ensure timely follow-up and return to primary care.
- Use clinician-approved standardized procedures to order labs and preventive screenings and refill routine medications.
- Coordinate care closely with other team members to identify patient self-care goals and ensure that medical and psychosocial needs are met.
- Acts as a resource for support staff.
- Facilitate follow-up care as needed.
- Current, unencumbered license to practice as a Registered Nurse in California.
- Ability to effectively coordinate care with triage assistants and other support staff, encouraging and nurturing development and growth, to build a strong and productive team.
- Strong organizational, administrative and problem-solving skills, and ability to be flexible and adaptive to change.
- Ability to effectively present information to other team members.
- Ability to seek direction/approval on essential matters, yet work independently with little onsite supervision, using professional judgment and diplomacy.
- Work in a team-oriented environment with several professionals with different work styles and support needs.
- Excellent interpersonal, verbal, and written skills.
- Conduct oneself in internal and external settings in a way that reflects positively on Life Long Medical Care as an organization of professional, confident and empathetic staff.
- Associate's or Bachelor’s Degree in Nursing.
- Current and valid California Registered Nurse license and BLS.
- Proficient in Microsoft Office suite.
- Electronic health records, EPIC preferred.
- A minimum of 1 year of experience as a Life Long Medical Nurse or 2 years of experience as a Registered Nurse.
- Ideally bilingual in Spanish.
- Ideally holds a California Public Health certification.
