We are looking for a Nurse Practitioner to join our fast‑growing, dynamic team. The role involves comprehensive health assessments including history taking, physical examination, medication review, home safety review, documentation for STAR and HCC measures, and medication prescription/order within scope.

• Obtain history and perform physical examination
• Complete review of medicines and related instructions
• Review home safety and psychosocial needs
• Fulfill documentation needs for STAR and HCC measures
• Document all pertinent information in patient charts and case management notes on same day as patient is seen; include medications and end‑of‑life preferences and drivers of preventable hospital admissions
• Instruct patients, caregivers and families to call before accessing medical care, except for life‑threatening emergencies
• Maintain patient safety and confidentiality
• Apply universal precautions in all patient care
• Prescribe/order procedures, treatments and medications within scope of practice
• Develop high level of trust and intimacy with patients and families

• California Licensed Nurse Practitioner
• Minimum 1 year experience in Primary Care

Base Salary: $135,000 - $155,000 per year.

Office Environment. Roles involve part to full time schedule in office environment. Sedentary activity level, majority of workday sitting at desk.
