Keep Me Home , a privately owned, Connecticut‑based home health agency with more than 40 years of experience, is seeking experienced and compassionate RNs and LPNs to expand our in‑home care team.

Local visits and time points are available within approximately 15 miles of the above towns.

• Valid, active RN or LPN license in Connecticut.
• Valid CPR certification.
• Driver's license and reliable transportation.
• Strong clinical judgment, communication, organizational, and time‑management skills.
• Comfortable with technology and electronic health records.
• Previous home‑health experience preferred but not required.
• Familiarity with Medicaid, Medicare, and Sandata EVV a plus.
• Bilingual Spanish‑speaking clinicians encouraged (salary incentives available).

• Perform complete patient assessments and develop individualized, evidence‑based care plans.
• Reassess and update care plans as patient needs change.
• Provide hands‑on nursing care, administer medications safely, and educate patients and family members.
• Coordinate care with interdisciplinary team members and community resources.
• For RNs: manage an assigned caseload as the primary case manager and oversee clinical outcomes.

Salary/Compensation: $30.00 – $60.00 per hour.
