Keep Me Home , a privately owned, Connecticut‑based home health agency with more than 40 years of experience, is seeking experienced and compassionate RNs and LPNs to expand our in‑home care team.
Locations- Bridgeport
- New Haven
- Waterbury
- New Britain
- Meriden
- Hartford
Local visits and time points are available within approximately 15 miles of the above towns.

- Valid, active RN or LPN license in Connecticut.
- Valid CPR certification.
- Driver's license and reliable transportation.
- Strong clinical judgment, communication, organizational, and time‑management skills.
- Comfortable with technology and electronic health records.
- Previous home‑health experience preferred but not required.
- Familiarity with Medicaid, Medicare, and Sandata EVV a plus.
- Bilingual Spanish‑speaking clinicians encouraged (salary incentives available).
- Perform complete patient assessments and develop individualized, evidence‑based care plans.
- Reassess and update care plans as patient needs change.
- Provide hands‑on nursing care, administer medications safely, and educate patients and family members.
- Coordinate care with interdisciplinary team members and community resources.
- For RNs: manage an assigned caseload as the primary case manager and oversee clinical outcomes.
Salary/Compensation: $30.00 – $60.00 per hour.
