• Administer medications and treatments per physician orders, within the scope of practice for an LPN.
• Monitor residents’ conditions, perform vital signs, and document findings in the electronic health record.
• Conduct head‑to‑toe assessments and provide essential nursing care to residents.
• Assist with activities of daily living and maintain resident safety.
• Administer injections and provide routine wound care as directed.
• Collaborate with registered nurses and the care team to implement and update care plans.

• Active LPN license in good standing; eligible to practice in the state of New Hampshire.
• Current CPR/BLS certification from a recognized provider.
• LPN diploma or certificate from an accredited program.
• Experience in a long‑term care setting is preferred.
• Ability to work in a travel assignment with shift flexibility as assigned.
• Willingness to undergo background check and licensure verification.
