Northgate Health & Rehab is seeking an experienced and detail-oriented MDS Coordinator (RN) to join our clinical leadership team. This is an excellent opportunity for a skilled nursing professional who understands the importance of accurate assessments, regulatory compliance, reimbursement optimization, and interdisciplinary collaboration.

The MDS Coordinator plays a critical role in ensuring the accuracy and timeliness of resident assessments while supporting quality outcomes and the financial success of the facility.

Responsibilities
  • Coordinate and complete MDS assessments in accordance with CMS, Medicare, Medicaid, and state regulations
  • Ensure timely and accurate completion of MDS, CAA, and care plan processes
  • Collaborate with nursing, therapy, dietary, social services, and other interdisciplinary team members
  • Monitor reimbursement and case mix opportunities
  • Review documentation to ensure accuracy and regulatory compliance
  • Participate in Medicare meetings and utilization reviews
  • Assist with survey preparedness and regulatory compliance initiatives
  • Analyze quality measures and support performance improvement efforts
  • Maintain knowledge of current MDS guidelines and reimbursement practices
Qualifications
  • Active Alabama RN license preferred
  • Previous MDS Coordinator experience in a Skilled Nursing Facility (SNF) preferred
  • Knowledge of MDS 3.0, RAI process, Medicare, Medicaid, and PDPM
  • Strong assessment, analytical, and documentation skills
  • Experience with electronic medical records and MDS software
  • Excellent communication and organizational abilities
  • Ability to work independently while collaborating with interdisciplinary teams
