Northgate Health & Rehab is seeking an experienced and detail-oriented MDS Coordinator (RN) to join our clinical leadership team. This is an excellent opportunity for a skilled nursing professional who understands the importance of accurate assessments, regulatory compliance, reimbursement optimization, and interdisciplinary collaboration.
The MDS Coordinator plays a critical role in ensuring the accuracy and timeliness of resident assessments while supporting quality outcomes and the financial success of the facility.

- Coordinate and complete MDS assessments in accordance with CMS, Medicare, Medicaid, and state regulations
- Ensure timely and accurate completion of MDS, CAA, and care plan processes
- Collaborate with nursing, therapy, dietary, social services, and other interdisciplinary team members
- Monitor reimbursement and case mix opportunities
- Review documentation to ensure accuracy and regulatory compliance
- Participate in Medicare meetings and utilization reviews
- Assist with survey preparedness and regulatory compliance initiatives
- Analyze quality measures and support performance improvement efforts
- Maintain knowledge of current MDS guidelines and reimbursement practices
- Active Alabama RN license preferred
- Previous MDS Coordinator experience in a Skilled Nursing Facility (SNF) preferred
- Knowledge of MDS 3.0, RAI process, Medicare, Medicaid, and PDPM
- Strong assessment, analytical, and documentation skills
- Experience with electronic medical records and MDS software
- Excellent communication and organizational abilities
- Ability to work independently while collaborating with interdisciplinary teams
