Job Shift: Indirect Care

• Facilitating smooth transitions through the continuum of care from preadmission to post discharge follow up (65%)
• Participates in meetings with the patient/family and interdisciplinary team (10%)
• Advocates for the patient and hospital for appropriate reimbursement with the insurance company(s), ensuring timely utilization management (5%)
• Informs patient/family of detailed insurance and benefit information (5%)
• Documents all communication with patient/family, insurance company and community partners in the electronic health record (10%)
• Other duties as assigned (5%)

• Education:
  
  Bachelor degree preferred.
• Experience:
  
  1-2 years of case management experience preferred; 2-3 years of clinical experience with medically complex patients preferred.
• License/Certification/Credential: LPN required; RN preferred.
