Hospice LPN – Full-Time/On-Call

Location:

South Hills, PA | 3 Assisted Living Facilities Only Flexible Schedule | Limited Travel

Overview Join Paramount Hospice as our dedicated On-Call Hospice Nurse serving residents within three Paramount Senior Living communities in the South Hills region only. This unique opportunity offers a highly manageable service area, strong facility support, and the ability to build meaningful relationships with residents and families while delivering exceptional hospice care. You will provide overnight hospice coverage from 5:00 PM to 8:30 AM, serving as the primary clinical resource for facility nurses, medication technicians, and caregivers.

Responsibilities include triaging urgent concerns, providing clinical guidance over the phone, coordinating care recommendations, responding to hospice death notifications, and making in-person visits when additional assessment, intervention, or support is needed. Most patient needs can be managed through collaboration and telephone triage with facility staff, allowing you to focus on delivering compassionate, high-quality hospice care while providing critical support when it matters most.

    5:00 PM to 8:30 AM
  • Approximately Two Evening Visits Per Shift as Needed
Qualifications
  • Active Pennsylvania LPN License
  • Hospice, Home Health, Long-Term Care, or Skilled Nursing Experience Preferred
  • Strong Clinical Assessment and Critical Thinking Skills
  • Excellent Communication and Documentation Abilities
  • Ability to Work Independently and Make Sound Clinical Decisions
  • Reliable Transportation for On-Site Visits When Necessary
