Position Title: LPN - Long-Term Care (LTC)
Location: Bethel, AK
Type: Full-Time/ Direct Hire
Weekly Hours: Approximately 40 Hours
Position Overview
The Licensed Practical Nurse (LPN) - Long-Term Care (LTC) provides direct nursing care to residents while supporting the delivery of high-quality, patient-centered care in a long-term care setting. This role collaborates with Registered Nurses, physicians, nursing assistants, and interdisciplinary teams to ensure residents receive safe, compassionate, and effective care that promotes optimal health outcomes and quality of life.
Responsibilities
- Provide direct patient care in accordance with individualized care plans and facility policies.
- Administer medications and treatments as prescribed and within scope of practice.
- Monitor resident conditions and promptly report changes to the healthcare team.
- Assist with resident assessments, care planning, admissions, transfers, and discharges.
- Maintain accurate and timely documentation of resident care and clinical observations.
- Collaborate with RNs, physicians, therapists, and other healthcare professionals to support resident outcomes.
- Provide patient and family education regarding treatments, medications, and care plans.
- Ensure compliance with facility policies, infection control practices, and regulatory requirements.
- Assist in supervising and supporting nursing assistants and ancillary staff.
- Promote a safe, respectful, and resident-focused care environment.
- Graduate of an accredited Practical Nursing (LPN) Program.
- Nursing Diploma or Certificate from an approved institution.
- Active Licensed Practical Nurse (LPN) License - Required.
- BLS Certification - Required.
- COVID-19 Vaccination Documentation - Required.
- Long-Term Care (LTC) experience preferred.
- Strong commitment to providing compassionate resident care.
- Excellent communication, documentation, and patient care skills.
- Previous Long-Term Care (LTC) experience.
- Experience working in skilled nursing or rehabilitation settings.
- New graduates and candidates with less than one year of experience may be considered.
