What You'll Do
  • Provide routine and PRN hospice visits in patients' homes, assisted living, memory care, and skilled nursing communities.
  • Perform comprehensive nursing assessments, symptom management, wound care, and medication education.
  • Support patients and families through every stage of the hospice journey.
  • Collaborate with physicians, social workers, chaplains, hospice aides, and the interdisciplinary team.
  • Complete accurate and timely EMR documentation.
What You'll Bring
  • Active Minnesota Registered Nurse (RN) license.
  • Hospice experience preferred, or strong experience in oncology, palliative care, home health, ICU, or end-of-life care .
  • Strong assessment, communication, and critical thinking skills.
  • Ability to work independently in a field-based role.
  • Reliable transportation and a valid driver's license.
