Registered Nurse opening in support of the National Intrepid of Center Excellence in Bethesda, MD caring for patients with traumatic brain injury.

Schedule:

• Monday - Friday, 40 hours
• No on-call

Qualifications:

• Possess an Associate degree from an accredited nursing program
• The qualified Registered Nurse will, have a minimum of 1 year nursing experience.
• Valid Registered Nursing license from any US jurisdiction
• Basic Life Support certification (BLS) from an American Heart Association Program or the American Red Cross

• Participate in all phases of care coordination.
• Provide nursing expertise about the coordination of care process, including assessment, planning, implementation, coordination, and monitoring. Identify opportunities for CM and identify and integrate local processes.
• Develop and implement local strategies using inpatient, outpatient, onsite and telephonic follow-ups.
• Develop and implement tools to support case management, such as those used for patient identification and patient assessment, clinical practice guidelines, algorithms, Care Point and MHS Genesis software, and databases for community resources.
• Integrate coordination of care and utilization management (UM) and integrating nursing case management with social work case management.
• Maintain liaison with appropriate community agencies and organizations.
• Accurately collect and document patient care data.
• Develop treatment plans including preventive, therapeutic, rehabilitative, psychosocial, and clinical interventions to ensure continuity of care toward the goal of optimal wellness.
• Establish mechanisms to ensure proper implementation of patient treatment plan and follow-up post discharge in ambulatory and community health care settings.
• Provide appropriate health care instruction to patient and/or caregivers based on identified learning needs.
• Function as a member of an interdisciplinary team to meet individual and family comprehensive health needs.
• Duties involve planning, booking and coordinating appointments, facilitating and coordinating meetings, and evaluating health care, serving as an advocate for patient options and services.
• Utilize an electronic health record for medical documentation.
• Participate in the completion of clinic requirements in regard to hospital directives and Joint Commission.
