Location: Saginaw, MI (also hiring in Grand Ledge, MI | Chambersburg, PA | Hershey, PA | Selinsgrove, PA)
Job Type: Full-Time
Compensation: Pay Per Visit (PPV) — rates based on visit type and home health experience
About the RoleA well-established home health and hospice organization is seeking a compassionate Physical Therapist to join its care team across multiple territories in Pennsylvania and Michigan. This is a full-time, community-based role for PTs who enjoy working independently, building relationships with patients in their homes, and being part of a collaborative clinical team that genuinely cares about outcomes.
Key Responsibilities- Deliver skilled physical therapy services to patients in residential home settings
- Develop and implement individualized care plans in coordination with the broader care team
- Complete clinical documentation accurately and on time using EMR software
- Manage your own schedule and caseload within the assigned territory
- Travel locally to patient homes — approximately 20% of your day is driving
- Bachelor's or Master's degree in Physical Therapy from an accredited program
- Active PT license in the state where you'll be working (PA or MI)
- Minimum 1 year of healthcare experience (new grads may be considered)
- Valid driver's license and reliable transportation
- Experience with EMR documentation preferred (Home Care Home Base a plus)
- Ability to self-manage time, schedule, and meet charting deadlines
- Flexible Monday–Friday schedule with 1 weekend every 4–6 weeks
