UPMC’s Western Behavioral Health’s Community Treatment Team (CTT) provides intense outpatient mental health treatment services to consumers with serious and persistent mental illnesses who have not previously found success in traditional outpatient mental health services.
Our clinicians provide community-based, evidence-driven care for individuals with severe mental illness, offering support through assessments, crisis response, and outpatient coordination. This hire for this role must be willing to work independently in a community based setting. This role is dynamic and rewarding, with opportunities for growth, teamwork, and professional development. As the first licensed ACT team in PA, our program is recognized as “Exemplary” in annual reviews.
This position is eligible for the Behavioral Health Fellows Program
—an initiative sponsored by Allegheny County Department of Human Services (DHS) in partnership with Community Care Behavioral Health and Jewish Healthcare Foundation. Designed to strengthen the County’s public behavioral health system, this program offers incredible benefits, including up to $45,000 in educational loan repayment (based on your degree level), or free access to an accredited professional certificate program, as well as paid time off for training and professional development as part of a cohort group.
Client Support & Case Management
- Coordinate care for a caseload of 10–15 individuals/families.
- Support goal setting, crisis planning, and WRAP development.
- Provide counseling, advocacy, and follow‑up services in community and office settings.
- Assist with access to food, healthcare, education, employment, and housing.
- Facilitate small groups to build coping and daily living skills.
- Conduct crisis assessments and manage emergencies.
- Collaborate with ER/DEC staff to divert from inpatient care.
- Work with team and community partners to coordinate services.
- Maintain accurate records and meet documentation deadlines.
- Ensure compliance with institutional and ACT model standards.
- Meet a 55% productivity rate.
- Participate in on‑call and weekend rotations.
- Apply motivational interviewing and evidence-based practices.
- Engage in ongoing ACT training and implementation.
