In-Home Nurse Practitioner or Physician Assistant - Scranton

Overview

Signify Health, part of CVS Health, is seeking a full‑time Clinician (Nurse Practitioner or Physician Assistant) to deliver in‑home health evaluations for health plan members. The role focuses on building trusted relationships and providing personalized care directly in patients’ homes.

• Conduct 45‑minute in‑home visits, including medication and medical history review, physical assessment, and ordering diagnostic tests as needed.
• Use an iPad and intuitive clinical workflow; not required to prescribe medication or alter current treatment regimens.
• Travel locally and within licensed states; conduct virtual visits as required.
• Provide Diagnostic & Preventive Services (e.g., spirometry, diabetic eye exams, bone density screening).

• Active, unrestricted license(s) in the coverage area(s). Board certification required; multi‑state licenses preferred.
• Ability to travel to various home environments and safely perform duties in homes with pets, second‑hand smoke, or allergens.

Primary workplace is the private residence of health plan members. Clinicians may encounter diverse settings and must be comfortable working in such environments.

Typical pay range: $87,035.00 – $ (base salary depends on experience, education, geography).
