** Position Summary
** The Registered Nurse (RN) provides professional nursing services to individuals with Intellectual and Developmental Disabilities (IDD) in home and community-based settings throughout Pennsylvania. The RN delivers skilled clinical care, implements individualized support plans (ISPs), conducts assessments, administers medications and treatments, and supports individuals and families in maintaining health, safety, dignity, and quality of life.
** Position Scope
** This position operates within the Nursing Department and reports to the RN Case Manager, Director of Nursing, or designated clinical leadership. The RN provides direct clinical services and collaborates with interdisciplinary teams to ensure compliance with Pennsylvania regulatory standards and One Well Health Care policies. Services are delivered in participant homes and approved community settings.
** Key Responsibilities
** The Registered Nurse (RN) provides skilled nursing care to individuals with Intellectual and Developmental Disabilities (IDD) in home and community settings. Responsibilities include administering medications and treatments, performing clinical procedures (such as wound care and vital sign monitoring), conducting assessments, supporting chronic condition management, documenting care accurately, responding to medical concerns, and maintaining infection control standards. The RN collaborates with interdisciplinary teams, educates families and caregivers, participates in case reviews, and supports quality and compliance initiatives to ensure safe, coordinated, and person-centered care.
