Nurse Practitioner / Physician Assistant – Wound Care

$140,000 Base + Performance Bonus | Monday–Friday | No Nights or Weekends

Are you an experienced Nurse Practitioner (NP) or Physician Assistant (PA) interested in specializing in wound care?

You'll provide specialized wound care to patients primarily in skilled nursing, long-term care, rehabilitation, and assisted living facilities throughout your assigned territory.

You'll independently manage your patients while working closely with facility clinical teams, physicians, patients, and families.

Responsibilities include:

• Performing comprehensive wound assessments
• Developing individualized treatment plans
• Providing bedside wound care
• Performing appropriate wound care procedures within your scope of practice
• Monitoring healing and adjusting treatment plans
• Educating patients and facility staff on wound treatment and prevention
• Collaborating with physicians, nurses, facility leadership, patients, and families
• Maintaining timely clinical documentation
• Traveling between assigned partner facilities

• Active Pennsylvania Nurse Practitioner or Physician Assistant license
• Ideally 2+ years of clinical experience
• Experience in SNF, long-term care, rehabilitation, assisted living, urgent care, geriatrics, or post-acute care preferred
• Wound care experience is a plus, but not required
• DEA registration or ability to obtain
• Comfortable working independently and traveling between assigned facilities
• Strong communication and clinical decision-making skills

This is a field-based position covering Franklin, York, and Cumberland Counties in Pennsylvania
.

Providers generally travel within an assigned territory of approximately a 55-mile radius
, with mileage and eligible tolls reimbursed
.

You don't need to already be a wound care specialist.
