Drug & Alcohol Case Management Specialist; Local Government

The Drug and Alcohol Case Management Specialist role with the Lackawanna-Susquehanna D&A offers an opportunity to make a positive difference every day. This position provides direct case management services for individuals with drug and alcohol needs. The role follows established policies and relies on sound judgment to support consumers throughout their care. As a Drug and Alcohol Case Management Specialist, you will perform the following duties:

• Consumer Screening and Assessment:
  Complete drug and alcohol screening and level of care assessments for all referred consumers
• Care Planning:
  Develop service plans and link consumers with community resources to meet determinants of health, and providers to implement the recommended level of care
• Treatment Monitoring:
  Track treatment progress and conduct required follow-up as desired by consumer, and document in required health record systems
• Support County Programs:
  Support multiple specialty and problem-solving court programs
• Provider Coordination:
  Liaison with providers to ensure coordinated services

• Full-time employment
• Work hours are 8:30 AM to 4:00 PM, Monday - Friday, with a 30-minute lunch.

• One year as a Drug and Alcohol Case Management Specialist Trainee; or
• A Bachelor's Degree in Chemical Dependency, Sociology, Social Welfare, Psychology, Nursing or a related field; and one year direct service experience performing counseling or case management work in alcohol or other drug treatment setting; or
• A bachelor's degree which includes or is supplemented by successful completion of 18 college credits in sociology, social welfare, psychology, criminal justice or other related social sciences; and one year direct service experience performing counseling or case management work in alcohol or other drug treatment setting; or
• Any equivalent combination of experience and training.

• Hiring preference for this vacancy may be given to candidates who live within Lackawanna and Susquehanna counties. If no eligible candidates who live within Lackawanna and Susquehanna counties apply for this position, candidates who reside in other counties may be considered.
