The Field Nurse in the Home Care Division at NYU Langone Health provides safe, quality nursing care using evidence-based practices. This role is ideal for nurses who excel in clinical decision-making and patient education while coordinating care with an interdisciplinary team.

• Facility: NYU Langone Hospital—Long Island
• Department/Unit: Home Care Division
• Schedule: Full-Time, Day shift
• Sign-On Bonus: $5,000

• Completes a comprehensive nursing assessment including physical, functional, psychosocial, and other factors.
• Derives diagnoses and needs from assessment data and collaborates to develop an individualized plan of care.
• Coordinates care by interacting with other healthcare team members to optimize patient outcomes.
• Delivers direct nursing care, ongoing assessment, teaching, and training to patients and caregivers.
• Identifies deficits and barriers to effective patient education and employs strategies to optimize engagement.
• Maintains accurate and timely documentation reflecting assessments, diagnoses, and plans of care.
• Assists Agency leadership in achieving corporate goals and improving patient care outcomes.
• Participates in quality assessment and performance improvement initiatives.

• Education: Bachelor's degree in Nursing required
• Experience: 1-2 years in Acute care facility or Home Health, Hospice, and Long Term Home Health Care Program
• Licenses &
  
  Certifications:
  
  Current Registered Professional Nurse in New York;
  Basic Life Support Certification through the American Heart Association
