Position Summary: LPN – Homecare, Eastern Long Island (Visiting Nurse / Multiple patients per day). This role utilizes clinical experience to plan, implement, and evaluate safe, therapeutic, age-appropriate patient care. The LPN works with patients and families at the center of care, collaborates with a multidisciplinary team, and performs both clinical and clerical duties while complying with hospital policies, regulations, and HIPAA guidelines.
Job Responsibilities- Complete documentation reflecting multidisciplinary coordination and assess patient response to nursing and team interventions.
- Evaluate patient progress toward health goals and modify care plans as needed; collaborate with RN to adjust plans.
- Recognize and respond to patient issues, escalating appropriately; ensure advance care directives are verified and documented.
- Maintain open communication channels through intra- and inter-shift reporting; demonstrate professional telephone etiquette.
- Respond to emergency situations, ensuring emergency equipment is functional and that HEICS plans are followed.
- Obtain prior authorization for medication and referrals for tests as required by insurance; keep exam rooms stocked and clean.
- Process medication orders and refills through the Workflow system, verifying dosage and medication accuracy.
- Implement conflict resolution skills for dissatisfied patients and document family advance care directives and rights.
- High School diploma or equivalent technical diploma.
- Graduate of an accredited school of licensed practical nursing.
- 0–1 year nursing experience in a hospital, ambulatory, private practice, or home health setting preferred.

Salary InformationSalary range: $76,552.53 – $88,418.23 annually, depending on experience, specialty, and education. This range does not include bonuses, incentive pay, or other compensation.
