Licensed Social Worker

Ledgewood Rehabilitation & Skilled Nursing Center in Beverly is seeking a compassionate and motivated Social Worker to join our team for 16 hours per week. This is a great opportunity for someone who values relationship-based care and wants the flexibility of a part-time schedule while supporting residents and families through every stage of their journey.

• Must possess a B.A in Social Work, LSW or equivalent degree AND a current social work license.
• Masters in Social Work (MSW), and LCSW or LICSW higher preferred.
• 1+ years skilled nursing facility experience preferred

• Compiles social history data.
• Participates in assessments, development and evolution of resident's care plan.
• Responsible for planning and leadership interdisciplinary care planning weekly meetings.
• Documents timely progress notes according to state and federal regulations.
• Coordinates the discharge planning for residents.
• Able to interpret and implement established social service policies, procedures and regulations.
• Coordinates services of community agencies for continued delivery of care upon discharge.
