* Job Description:

** A member of the DMH Flexible Support Services team to provide non-clinical support to individuals who are involved in Flexible Support Services.  Serve as a recovery guide and role model in the management of recovery and assist the individual to identify and overcome barriers to recovery, connect the individual with services and encourage hope, optimism and health. Provide support with problem solving and advocacy to assist individuals to meet their goals.

Provide education to individuals and family members on the recovery process.
** Responsibilities:
*** Meet with adolescents who present with significant emotional, mental, and behavioral challenges, and their families, providing individual, team, or group interventions.
* History of lived experience including a psychiatric diagnosis or experience with emotional distress or trauma resulting in significant life disruption. Willingness to share their personal story of recovery with peers and persons served to inspire hope and motivation.
* Complete Required Training – Level 1:
Complete Core Elements of Young Adult Peer Mentoring Training prior to hire (or within first 3-6 months with approval from program funding source). Level 2:
Complete CPS training certification within first 6 months of hire (applicable for Senior Young Adult Peer Mentors only).
* Be knowledgeable about the mental health system and link the individual to treatment and  resources including support groups, clinical services, and community resources/supports.  Links young adults to peer support services such as Young Adult Resource Centers.
* Collaborate with youth, family, treatment providers, school and other supports to ensure continuity of care with other services and to ensure that young adult peer mentoring services are integrated and clinically coherent. Collaboration on goal development, crisis planning, and problem-solving in partnership with youth, parent/caregiver, and other support persons as needed. Authorizations are obtained by provider as necessary.
* Provide telephonic (or text) or in-person follow-up support, coaching, and assistance to help the individual access treatment or services.
* Demonstrate an understanding and belief in the recovery process and Recovery Oriented Systems of Care Model. Promote self-care through modeling of healthy habits and sharing resources/strategies that have been helpful to the young adult peer mentor and may be useful to the youth served.
* Knowledge of basic crisis intervention, Motivational interviewing, and case management techniques preferred. Support youth in identifying their strengths, vision, and goals.
* Demonstrates strong boundaries regarding confidentiality and personal relationships, while at the same time be able to share personal knowledge about recovering from and managing a mental health challenge. Role model and support youth on discerning how to share lived experience effectively and safely in order to support youth in safely sharing their own experiences.
* Support youth to develop effective communication with family, friends, allies/supports, providers and others. Work with the youth’s team or other supports to ensure that youth’s vision and goals guide treatment decisions, focus on aspirational life goals, and support youth in developing skills that they need.
* Provides temporary assistance with transportation as needed to essential behavioral health appointments while transitioning to community-based transportation resources (e.g., public transportation resources, PT-1 forms, etc.).
* Complete training provided by agency and participates in ongoing supervision.
* Ensures that all written and billing documentation is complete in a timely manner.
