Responsibilities
  • Primarily provide outpatient rehabilitation services in an in-home setting for the older adult population.
  • Evaluate and treat patients in the most functional environment, typically the patient's home.
  • Assess, develop, and deliver physical therapy programs to facilitate rehabilitation.
  • Identify and make referrals to other disciplines to address comprehensive needs.
  • Implement individualized therapy programs designed to restore, reinforce, and enhance performance.
  • Instruct, educate and train patients and caregivers in essential skills and functions to promote independence and productivity.
  • Direct activities to promote and maintain health.
  • Distribute flexible schedule across various in-home settings.
  • Supervise Physical Therapy Assistants, temporary licensees, students and support personnel in accordance with state licensure requirements and professional standards.
  • Ensure adherence to department and patient treatment schedules, timely clinical and billing documentation, and work-related meetings.
  • Attend and contribute to patient care, staffing conferences and other related meetings.
  • Consult with other members of the rehabilitation team to select the most appropriate therapeutic regimen.
  • Adapt program and/or plan of care according to the needs of the individual patient.
  • Promote a safe environment and enforce the elimination of fire and safety hazards.
  • Order supplies and equipment as necessary.
  • Clean, maintain, identify, and report equipment repair needs.
  • Ensure patients and families receive the highest quality of service and care.
  • Screen, examine, and evaluate patients, including history, systems review, and appropriate tests.
  • Establish diagnosis, identify impairments, determine predicted improvement levels and time needed to achieve it.
  • Identify precautions/contraindications and design a plan of care.
  • Develop appropriate treatment goals and methods in collaboration with patients and caregivers.
  • Implement the physical therapy treatment plan and complete related documentation and record keeping.
  • Provide services including:
    • Therapeutic exercise: aerobic conditioning, balance coordination, agility training, body mechanics, postural stabilization, flexibility, gait and locomotion training, neuromotor development, relaxation, strength, power, endurance.
    • Functional training
    • Manual therapy
    • Airway clearance techniques
    • Integumentary repair and protection
    • Use of electrotherapeutic, physical agent and mechanical modalities
    • Health and Wellness
    • Education on Chronic Disease Management
    • Falls Assessment and interventions to reduce fall risk
    • Case management in the home and community
  • Design, fabricate, fit, and/or apply adaptive devices, and train patients in their use.
  • Select appropriate activities suited to the individual's physical and mental capacity, intelligence level and interest.
  • Instruct patients in skills and techniques required for participation in mobility activities and evaluate progress.
  • Perform routine reexamination as needed to modify/progress treatment plan.
  • Incorporate health and wellness programming into treatment plans.
  • Perform home and community mobility assessments and provide safe mobility recommendations.
  • Perform other related duties as required.
Qualifications
  • Must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility.
  • Must have a Master's degree in Physical Therapy; or
  • Must have a Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy; or
  • Must have a Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience.
  • If the Physical Therapy degree earned outside of the U.S., must be evaluated to be the U.S. equivalent to a degree in Physical Therapy.
  • Must have a valid driver's license or reliable, independent transportation to and from the patients' residence.

Posted Salary Range: USD $63.00 - USD $63.00 /Hr. Bonus: USD $5,000.00
