Longwood Hospice | North Shore, MA

Longwood Hospice is seeking a compassionate and experienced Per Diem Hospice Chaplain to provide spiritual and emotional support to hospice patients and their families.

This is a field-based per diem position serving hospice patients residing in our skilled nursing facilities (SNFs) on the North Shore of Massachusetts. The Chaplain will travel to our North Shore facilities to visit patients and work closely with families, facility staff, and the Longwood Hospice interdisciplinary team.

Responsibilities
  • Visit hospice patients residing in our North Shore skilled nursing facilities to provide individualized spiritual and emotional support.

  • Assess the spiritual needs, beliefs, values, and preferences of patients and families.

  • Develop and implement individualized spiritual care plans as part of the hospice Plan of Care.

  • Provide compassionate support to patients and families facing end-of-life concerns, grief, loss, and other emotional or spiritual needs.

  • Respect and support patients of all faith traditions, spiritual beliefs, and those who do not identify with a particular faith.

  • Collaborate with nurses, social workers, physicians, facility staff, and other members of the hospice interdisciplinary team.

  • Participate in interdisciplinary team (IDT) meetings as needed.

  • Provide prayer, spiritual counseling, rituals, or other appropriate support based on each patient's individual wishes.

  • Support family members and caregivers throughout the hospice journey.

  • Assist with bereavement support when appropriate.

  • Complete timely and accurate documentation of patient visits and spiritual care services.

  • Maintain patient privacy and confidentiality in accordance with HIPAA and Longwood Hospice policies.

Qualifications
  • Education or training in theology, ministry, pastoral care, chaplaincy, or a related field.

  • Previous experience providing spiritual or pastoral care strongly preferred.

  • Hospice, palliative care, skilled nursing, or healthcare experience preferred.

  • Ability to provide compassionate, nonjudgmental support to individuals from diverse religious, spiritual, and cultural backgrounds.

  • Excellent interpersonal and communication skills.

  • Ability to work independently while collaborating with an interdisciplinary healthcare team.

  • Reliable transportation and ability to travel to skilled nursing facilities throughout the North Shore required.
