LPN Care Coordinator
39 PTO Days | No Weekends
Full Time | PACE Program | Senior Care Coordination
POSITION OVERVIEW- PACE-focused LPN coordination role supporting older adults within outpatient and home-based care environment
- Opportunity to build long-term relationships while coordinating care and supporting patient independence
- Primarily clinic-based role with occasional home visits as needed
- Collaborative interdisciplinary environment working closely with Occupational Therapy and nursing teams
- Autonomous role assessing patient needs and recommending care plan updates
- Monday-Friday 8:00 AM-4:00 PM schedule
- No weekends or call responsibilities
- 39 combined PTO and holiday days annually

- Strong interdisciplinary collaboration
- Stable outpatient structure with patient relationship continuity
- Mileage and license reimbursement included
- Monday-Friday
- 8:00 AM-4:00 PM schedule

- Salary range: $67,000-$80,000 annually

- Strong communication and interdisciplinary collaboration skills REQUIRED
- Comfortable with outpatient coordination and occasional home visits REQUIRED
