Full Time | PACE Program | Senior Care Coordination

POSITION OVERVIEW
  • PACE-focused LPN coordination role supporting older adults within outpatient and home-based care environment
  • Opportunity to build long-term relationships while coordinating care and supporting patient independence
  • Primarily clinic-based role with occasional home visits as needed
  • Collaborative interdisciplinary environment working closely with Occupational Therapy and nursing teams
  • Autonomous role assessing patient needs and recommending care plan updates

SCHEDULE
  • Monday-Friday

  • Salary range: $67,000-$80,000 annually

ROLE RESPONSIBILITIES
  • Coordinate home care and outpatient support services for geriatric patient population
  • Assess patient medical and functional changes
  • Recommend updates to patient care plans
  • Collaborate with Occupational Therapy and interdisciplinary care teams
  • Support continuity of care and patient independence goals
  • Conduct home visits as needed within care coordination structure

REQUIREMENTS
  • Active Massachusetts LPN license REQUIRED

  • Strong communication and interdisciplinary collaboration skills REQUIRED
  • Comfortable with outpatient coordination and occasional home visits REQUIRED
