Hybrid RN Case Manager | No Bedside | No Weekends

Full Time | Senior Care Coordination | Community-Based Nursing

POSITION OVERVIEW
  • Hybrid RN Case Manager role supporting aging adults through community-based care coordination and individualized care planning

SCHEDULE
  • Monday-Friday schedule
  • 8:00 AM-4:00 PM hours
  • Three community-based days weekly

COMPENSATION & BENEFITS
  • Base salary: $84,000-$108,000 annually based on experience

ROLE RESPONSIBILITIES
  • Develop individualized patient care plans
  • Coordinate community-based services and continuity of care
  • Conduct patient visits within community and home settings
  • Manage follow-up and ongoing care coordination remotely
  • Collaborate with interdisciplinary care teams
  • Support aging adults living independently at home

  • Active Massachusetts RN license REQUIRED

  • Comfortable with hybrid remote/community workflow REQUIRED
