Full Time | Senior Care Coordination | Community-Based Nursing
POSITION OVERVIEW- Hybrid RN Case Manager role supporting aging adults through community-based care coordination and individualized care planning
- Opportunity to work independently without bedside nursing demands
- Role focused on continuity of care, service coordination, and long-term patient support within home-based setting
- Hybrid structure combining community visits with remote care coordination responsibilities
- Mission-driven senior care environment with strong interdisciplinary collaboration
- Hybrid schedule with two remote workdays weekly
- No bedside nursing responsibilities
- No nights, weekends, or call
- Strong autonomy and independent workflow structure
- Meaningful relationship-focused work with older adults
- Stable weekday schedule with work-life balance
- Team-based support structure within established PACE organization
- Monday-Friday schedule
- 8:00 AM-4:00 PM hours
- Three community-based days weekly
- Two remote workdays weekly
- No nights
- No weekends
No call responsibilities
- Base salary: $84,000-$108,000 annually based on experience

- Active Massachusetts RN license REQUIRED
- Minimum 2 years RN experience REQUIRED
- Home care experience preferred

- Team-based care approach REQUIRED
- Comfortable with hybrid remote/community workflow REQUIRED
