• PACE Registered Nurse role focused on caring for older adults in outpatient and community-based settings

SCHEDULE

• Monday-Friday schedule

• 8:00 AM-4:00 PM hours

• No weekends

• No call responsibilities mentioned

COMPENSATION & BENEFITS

• Base salary: $80,000-$104,000 annually based on experience

• Health insurance with 75% employer-paid premium

ROLE RESPONSIBILITIES

• Provide direct nursing care to senior patients

• Complete initial and ongoing patient assessments

• Develop individualized care plans

• Support patients in maintaining safe independent living

• Collaborate with interdisciplinary care team

• Deliver care within clinic and community settings

Requirements

• Active Massachusetts RN license REQUIRED

• Minimum 2 years recent RN experience REQUIRED

• Comfortable providing care in clinic and home/community settings REQUIRED

• Strong assessment and care planning skills REQUIRED

• Team-based care approach REQUIRED

• Primary care or geriatrics experience preferred
