Position Summary

Oversees medication management and coordinates residents' medical needs with physicians, Director of Nursing/Resident Care, and family members. Observes, documents, and reports significant changes in residents’ condition, recommending care adjustments. Supervises Resident Care Aides to ensure proper care delivery.

Essential Duties & Responsibilities
  • Clinical Judgment and Decision Making: Participates in direct and indirect nursing care, health maintenance, teaching, counseling, collaborative planning, and rehabilitation.
    • Assess health status and related data.
    • Analyze and interpret health data to determine nursing care needs.
    • Plan and implement nursing interventions, incorporating current medical advancements.
    • Integrate prescribed medical regimens into care plans.
    • Educate patients and families on health and preventive care.
    • Evaluate outcomes and modify care plans as needed.
    • Ensure safe patient care through organizational skills.
    • Develop culturally competent care plans respecting patient values.
    • Deliver age-specific care.
  • Collaborative Relationship: Work with healthcare providers, patients, and families to ensure quality care.
    • Establish collaborative relationships.
    • Educate about illness and treatment.
    • Support community-based activities.
  • Clinical Leadership: Responsible for the quality of nursing care provided under their direction.
    • Adhere to policies and practice guidelines.
    • Be a responsive team member and educator.
    • Maintain regulatory compliance.
  • Clinical Scholarship: Achieve and uphold standards of quality nursing care, committed to lifelong learning.
    • Uphold care standards.
    • Engage in self-reflection and education.
  • Qualifications
    • Graduate of an accredited School of Nursing.
    • Minimum 1 year of medical/surgical or skilled nursing experience.
    • Current nursing license from the Massachusetts Board of Registration.
    • BLS certification.
