Market leading healthcare organization looking for a Psychiatric Mental Health Nurse Practitioner to join our team as a member of the Interdisciplinary Team! Very competitive salary, robust benefit plan, and quality of life working Monday to Friday 8‑am to 4‑pm. The position is full‑time with no nights, weekends, or holidays, covering Beverly and Gloucester sites.

Responsibilities
  • Perform comprehensive initial behavioral health evaluations for complex older adults in a variety of care settings including day center, home, assisted living facilities, or skilled nursing facilities.
  • Evaluate participants for acute changes in behavioral health status to mitigate risk of psychiatric hospitalization and provide close follow‑up after hospitalizations.
  • Make nursing home visits in accordance with state nursing home requirements.
  • Make home visits to participants as needed.
  • Follow up with participants routinely or urgently as clinical need arises.
  • Develop clinically appropriate behavioral health treatment plans and regularly review medication regimens.
  • Implement gradual dose reductions (GDR) as needed in line with regulatory practices.
  • Monitor patients' response to psychotropic medications and adjust treatment plans as warranted.
  • Document evidence‑based and clinically appropriate treatment interventions with a strong emphasis on psychopharmacology management.
  • Participate in behavioral health initiatives to enhance clinical practice, teamwork, and program development.
  • Cover for behavioral health colleagues when needed.
  • Provide appropriate behavioral health teaching to participants, families, caregivers, and the interdisciplinary team.
  • Adhere to the standards set forth in the NP/MD Collaborative Practice Guide.
  • Complete accurate and timely documentation.
  • Ensure duties are accomplished in a fiscally responsible manner while adhering to established budgetary constraints.
  • Ensure all interactions with participants are conducted with the highest standards of care for all clients.
  • Pass an N95 mask fit test; mask is required where the seal is critical.
  • Travel locally as required.
  • Perform other duties as required.
Qualifications & Skills
  • Master of Science in Nursing (MSN).
  • Current Board Certification as a Psychiatric Mental Health Nurse Practitioner.
  • Current licensure as a Nurse Practitioner in Massachusetts.
  • Current Massachusetts Controlled Substances Registration and DEA Prescription Privileges, current CPR Certification.
  • At least five (5) years of experience working with a geriatric population.
  • At least one (1) year of experience working with low‑income and diverse cultural groups preferred.
  • Previous skilled nursing facility experience preferred.
  • Obtain and maintain appropriate contracted facility credentials and privileges.
  • Demonstrate ability to work within urban communities with a strong emphasis on racially, ethnically, and culturally diverse, low‑income, and special‑needs populations.
  • Work well within a team and respect coworkers and colleagues.
  • Communicate clearly and succinctly with other team members.
  • Recognize situations and problems early and implement effective solutions.
