Description:

The Physical Therapist partners with residents and other care providers to provide physical therapy services.
The aim is to maximize residents' functional abilities and independence in their activities of daily living.
Essential duties include performing evaluative and therapeutic procedures under the order of a physician or healthcare provider.
Evaluations are performed per department protocols to establish a therapeutic plan of care.
This plan includes functional long- and short-term goals, frequency and duration of services, and modalities to be used.
Input from the resident and primary caregivers is sought regarding the therapeutic plan of care.
This information is incorporated into the therapy goals.
The Physical Therapist provides consultation to the referring physician and other members of the healthcare team.
They monitor and evaluate treatment effects and progress the treatment program toward functional goals.
Modifications to the treatment plan are made as necessary.
All evaluative and therapeutic interactions with the resident are documented in the Electronic Medical Record.
Documentation is done per department procedures and ensures accessibility to others on the care team.
The Physical Therapist recommends and obtains appropriate assistive devices and provides instruction on their use.
Appropriate education is given to the resident and/or caregiver, with results of teaching documented.
Residents are referred appropriately to Physical Therapist Assistants when necessary.
Collaboration with other therapy professionals ensures good resident care.
Compliance with Medicare and other reimbursement regulations is understood and followed.
In-service education regarding clinical knowledge is provided to nursing and other departments.
The Physical Therapist actively assists with the development of new programs and services in Rehabilitation.
Awareness of professional issues is maintained through membership in professional organizations and reading professional publications.
Performance issues are brought to the attention of the Director.
Mentorship is provided to new staff during orientation, ensuring completion of departmental procedures.
Department cleanliness and maintenance are ensured by keeping office and clinical areas tidy.
Routine cleaning of equipment and space is assisted.
All infection control procedures are followed.
Required licenses, certifications, and competencies must be maintained, with annual compliance courses completed on time.
Other duties consistent with this role may be assigned.
Must possess the physical and mental abilities to perform tasks associated with this position.
This involves a combination of sitting, standing, walking, bending, stooping, twisting, and reaching.
The ability to lift up to 50 pounds is required, with a two-person lift for over 50 pounds.
Lifting and carrying frequently up to 20 pounds is necessary.
Some stress may be related to a high level of responsibility for quality care.
Must be able to perform all essential functions of this position with or without a reasonable accommodation.
