GBMC Health Care is seeking a licensed social worker (LMSW/LCSW) to join our hospice team. The role involves assessing patients and families, developing care plans, and providing counseling around dying, coping, and planning.
Requires a Master’s in Social Work and MD licensure within 3 years. Two+ years of health care experience, hospice preferred. Will work with an interdisciplinary team in patients’ homes and care settings.
