RN Case Manager - Care Coordination and Discharge Planning Job Description

We are in need of an experienced RN Case Manager to join our team. This position focuses on Care Coordination and Discharge Planning in addition to Utilization Review. The department is currently operating with limited staffing, making this role critical for enhancing length of stay management efficiency.

• Coordinate patient care to ensure optimal outcomes.
• Execute discharge planning to enhance patient transitions.
• Conduct utilization reviews to optimize resource management.
• Work collaboratively with interdisciplinary teams to improve patient care quality.

This position offers a full-time schedule with shifts from 7:30 AM to 4:00 PM.

• Active RN license.
• Experience in case management or related fields.
• Strong understanding of care coordination principles.
• Familiarity with discharge planning processes.
