Position: Office Based Nurse Care Coordinator -  Part Time of Full Time

Office-Based BHH Nurse Care Manager (RN or LPN) – Part-Time or Full-Time

The Role & Schedule

  • Predictable

    Hours:

    Choose from part-time or full-time (20–40 hours per week) during standard Monday–Friday business hours.
  • True Balance:
    Zero nights, zero weekends, no holidays, and no on-call demands.
  • Collaborative Environment:
    Work daily alongside a multidisciplinary team—including a Clinical Lead, Case Managers, Peer Support Specialists, and Psychiatric Consultants—to support clients managing severe mental illness and complex physical health conditions.

Your Impact (Core Responsibilities)

  • Medical Advocacy & Coordination:
    Track health outcomes, translate medical data into actionable interdisciplinary plans using our EHR, and occasionally attend local doctor appointments with clients to provide in-person advocacy.
  • Clinical Mentorship:
    Serve as the primary medical resource for non-medical staff, providing vital health-literacy training and weaving physical health strategies into behavioral care plans.
  • Consultative Care:
    Guide clients transitioning out of hospitals or emergency rooms to ensure safe, successful discharges and complete accurate medication reconciliations.
  • Safety Advisory:
    Provide clinical guidance during crisis situations and advise AWS management on health, safety, and infectious disease protocols.

Compensation & Pay Range

  • Licensed Practical Nurse (LPN): $28.00 – $34.00 per hour
  • Registered Nurse (RN): $35.00 – $40.00 per hour

What You Bring (Qualifications)

  • Licensure:
    Active, RN or LPN license in the State of Maine (or a valid multi-state compact license).
  • Certifications:

    Current CPR/Basic Life Support (BLS) certification.
  • Mobility:
    Valid driver's license, clean driving record, and reliable transportation (essential for local client advocacy).
  • Skills:

    Exceptional communication, the ability to work autonomously, and comfort navigating standard office software and EHR systems.
  • Bonus Points:

    Experience in behavioral health, community health, or working with underserved populations managing comorbid conditions.

Qualifications & Requirements

  • Required:
    • Licensure:
      Active, LPN or Registered Nurse (RN) license in the State of Maine (or valid multi-state compact license).
    • Certifications:

      Current CPR/Basic Life Support (BLS) certification.
    • Mobility & Transportation:
      Valid driver's license, a good driving record, and reliable transportation (essential for providing client advocacy at local doctor's appointments).
    • Tech Fluency:
      Comfort navigating Electronic Health Records (EHR) and standard office software for data-driven care coordination.
    • Core Competencies:

      Exceptional communication and organizational skills, with the proven ability to work autonomously while seamlessly collaborating with a multidisciplinary team.
    • Compliance:
      Successful completion of all required background checks; must not be listed on state or federal suspension and disbarment lists (OIG).
  • Preferred:
    • Clinical

      Experience:

      Prior experience in behavioral health, community health, or working with underserved, transient populations managing both clinical and psychiatric comorbidities.
