Nurse Practitioner or Physician Assistant - Physical Medicine & Rehabilitation Company Overview

Total Extended Care Services is a growing Physical Medicine and Rehabilitation (PM&R) group dedicated to improving outcomes for patients in sub-acute and skilled nursing settings. Our team partners closely with rehabilitation professionals to help patients regain function, reduce hospital readmissions, and improve quality of life.

The Opportunity

This role offers the chance to work alongside experienced PM&R physicians in a highly collaborative, interdisciplinary environment. It's well-suited for a clinician who wants to focus on functional recovery, continuity of care, and meaningful patient outcomes--while maintaining a flexible schedule and no on-call responsibilities.

You’ll play a central role in helping patients progress through rehabilitation while working with a team that prioritizes coordination, consistency, and measurable impact.

What You’ll Do
  • Partner with a PM&R physician to manage patient care in skilled nursing and sub-acute rehab settings
  • Collaborate with therapy teams (PT/OT/ST) to align medical treatment with functional recovery goals
  • Coordinate care with primary care providers, nursing staff, social workers, and families
  • Manage pain and rehabilitation needs with a focus on improving function and reducing hospital readmissions
  • Ensure timely, accurate documentation, with flexibility to complete charting remotely

What We’re Looking For
  • Licensed Nurse Practitioner or Physician Assistant (required)
  • Active certification as an NP or PA (required)
  • Current Michigan license (required)
  • Strong communication and collaboration skills in a team-based care environment
  • Ability to work independently across multiple facilities
  • Reliable transportation
