Graham Healthcare Group is seeking a Registered Nurse (RN) Home Health for a nursing job in Big Rapids, Michigan.

• Deliver skilled, patient‑centered care in patients' homes.
• Perform patient assessments, medication management, and care planning.
• Educate and empower patients and families on disease management and treatment plans.
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation.
• Work independently while collaborating with an interdisciplinary team.
• Travel to patient homes within an assigned territory.

• Active Registered Nurse (RN) license in the state you work.
• At least one year of RN experience with the clinical expertise to deliver quality patient care.
• Current driver's license and the ability to spend approximately 20% of your day driving to/from patient locations.
