Description:

Delivers nursing care responsibly and accountably as described by professional standards. Demonstrates competency through the nursing process, which includes assessment, diagnosis, outcomes identification, planning, implementation, and evaluation. Upholds the standards of professional performance, including ethical practice and culturally congruent practice. Communicates and collaborates with other members of the healthcare team to ensure continuity and coordination of care. Acts as a patient and family advocate.

Performs point of care testing as required. Assesses the overall needs of the patient and analyzes assessment data to identify potential diagnoses. Identifies expected outcomes and develops an individualized plan of care. Evaluates patient and family educational needs and employs appropriate teaching strategies. Initiates patient discharge planning at the time of contact. Ensures that required documentation is accurate and timely.

Assumes a leadership role and delegates appropriately to facilitate safe, quality patient care. Engages in ongoing improvement activities that contribute to quality patient care. Actively participates in safety initiatives and completes all position-related safety competencies on time. Performs other duties as assigned.
